Tissue Expander to Implant Breast Reconstruction

Welcome to DR SAMARTH GUPTA

This is a two-stage implant-based reconstruction often performed after mastectomy, especially when patients don’t have enough tissue for flap-based reconstruction.

How It Works:

  1. Stage 1: Tissue Expander Placement
    A temporary expander is placed under the skin and chest muscle during or after mastectomy. It has a small valve that allows gradual saline injections over several weeks to stretch the skin and muscle safely.
  2. Stage 2: Permanent Implant Placement
    Once the chest has expanded enough, a second surgery is performed to replace the expander with a permanent implant—either saline or silicone gel, based on your preference and anatomy.

Is It Right for You?

This method is ideal for:

  • Patients lacking sufficient tissue for autologous flaps.
  • Those seeking a shorter recovery time and less invasive surgery.

Recovery Overview:

  • Expander placement usually involves a 1–2 week recovery.
  • Expansion phase takes 4–8 weeks with regular clinic visits.
  • Implant exchange is done as a day surgery, with most patients resuming normal activity within 2–3 weeks.

Direct-to-Implant Breast Reconstruction

Direct-to-Implant (DTI) reconstruction allows for the placement of a permanent breast implant immediately at the time of mastectomy, eliminating the need for a tissue expander and a second surgery.

Procedure Overview:

In a single-stage surgery, the breast implant is inserted right after the breast tissue is removed during mastectomy. This approach is typically performed in carefully selected patients and often includes use of an acellular dermal matrix (ADM) to provide additional support and coverage for the implant.

Ideal Candidates:

  • Small to moderate breast size
  • Minimal ptosis (droop)
  • Good skin quality and vascularity post-mastectomy
  • No prior radiation therapy
  • Non-smoker, with good general health
